Ian Milton

Founder

Ian brings over two decades of leadership, operational, and investigative expertise, forged during 24 years of service in the British Army. His career began at the prestigious Royal Military Academy Sandhurst and progressed to roles as a specialist advisor to UK Special Forces and the leader of teams on complex operations worldwide.
In the latter years of his military career, Ian specialised in accident investigation, acting as lead investigator for high-profile fatality cases. These investigations demanded a meticulous, evidence-based approach to determine root causes, uncover systemic failings, and implement safety improvements to prevent recurrence. In these high-stakes environments, Ian applied a “no stone left unturned” philosophy, ensuring every finding was backed by facts and every recommendation could stand up to the highest scrutiny.

Ian’s work in cases involving multiple fatalities brought him face-to-face with the human cost of operational incidents. This experience underlined for him that thorough investigation is not only technical — it is also human. The people involved, from witnesses to responders, often face significant psychological impact. To address this, Ian incorporates Trauma Risk Management (TRiM) into his investigative practice and training, ensuring teams can identify early signs of trauma, provide peer support, and maintain operational effectiveness throughout the process.

Now, Ian delivers specialist accident investigation training and TRiM practitioner programmes, equipping professionals to uncover true causes, safeguard team wellbeing, and embed sustainable safety improvements. He has also delivered leadership programmes and built high-performing teams internationally, combining technical rigour with his belief that relationships and trust are central to investigative success. Behind every incident, Ian reminds his teams, there is a human story that must be heard and understood.

Martin Grant

Martin is a highly experienced Accident Investigation and Safety Facilitator with a proven record of helping organisations strengthen their safety culture, improve investigative capability, and reduce operational risk. He specialises in designing and delivering practical, evidence-based training that equips professionals to conduct thorough investigations, identify root causes, and implement effective prevention measures.
Over his career, Martin has designed and delivered more than 1,000 workshops and training sessions on accident investigation, safety protocols, incident analysis, and behavioural safety for a wide range of industries. His approach ensures that participants not only learn the investigative process but also understand how to apply findings to drive meaningful, lasting safety improvements.

Martin works closely with clients to assess needs, tailor programmes, and evaluate outcomes — ensuring every course leads to measurable improvements in investigation quality and safety performance. His clear communication style and methodical problem-solving help organisations build investigative processes that stand up to both operational and legal scrutiny.

In addition to his accident investigation expertise, Martin holds a Master’s in Leadership, MCIPD accreditation, a Level 4 Certificate in Education & Training, and multiple professional coaching certifications, enabling him to integrate strong facilitation skills with deep technical knowledge.

For Martin, effective accident investigation is more than identifying what happened — it’s about uncovering why it happened, ensuring lessons are applied, and preventing future harm.

Martin is a seasoned risk professional with over 25 years of leadership experience in the Financial Services sector, specialising in risk assessments and fostering a proactive risk culture. He has overseen risk assessments across 19 global jurisdictions, ensuring the alignment of risk strategies with local and global priorities.

 

Previously, Martin held senior roles at the Royal Bank of Scotland, where he led large scale operational teams and implemented risk assessment programs and a global risk culture initiative, shifting the organization from reactive to proactive risk management.

Martin is committed to building teams that integrate risk into everyday decision-making and drive continuous improvement in risk management practices.

Griff Gurley

Griff is a highly experienced investigator and leader with over 24 years of service in the British Armed Forces, where he specialised in high-stakes environments requiring precision, accountability, and resilience.

A significant portion of his career was dedicated to leading investigations into accidents and near-miss incidents—often under intense pressure and in remote or austere conditions. These investigations were not just about identifying what went wrong, but about understanding why—examining behavioural patterns, decision-making processes, and environmental stressors. Griff’s approach consistently prioritised transparency, learning, and systemic improvement over blame.

His work has shaped a deep belief in risk management practices that go beyond compliance. He advocates for a culture where safety, performance, and leadership are interconnected—where insights from critical incidents drive meaningful change and operational excellence.

Today, Griff supports corporate leaders and organisations in strengthening their risk culture, improving incident response, and embedding learning into their operational frameworks. He brings a calm, analytical mindset and a commitment to helping teams lead with clarity, accountability, and confidence—even in the most challenging circumstances.
